Elevo Care is proud to work closely with NDIS Participants, families and hospitals to help you transition from hospital back to community.
Our experienced team inline with your NDIS Plan will:

work collaborately with you, your family members and broader support network, the hospital and accomodation providers, to manage your planned transition from hospital back to community

provide you with options of suitable accomodation

build a supportive and experienced team of support workers to provide quality person-centred supports in your new accomodation setting

support you to achieve your short-term, medium-term, and long-term goals
